fix: align input IDs with env var names in GitHub and Grafana definitions (#85)

- com.github-mcp-docker.json: GITHUB_TOKEN → GITHUB_PERSONAL_ACCESS_TOKEN
- com.grafana-mcp-docker.json: GRAFANA_TOKEN → GRAFANA_SERVICE_ACCOUNT_TOKEN

Input IDs should match the environment variable names they map to for
consistency and to avoid confusion when configuring servers.
